Team Harford Gymnastics Parent Agreement

Status message

A healthy facility starts at home. When you stay at home at the first signs and symptoms of a communicable disease, you are protecting yourself from additional health challenges. You are also helping protect other athletes/staff from exposure to the disease. The goal is for an individual to arrive well rested, healthy, and disease free to practice. We require all team and pre-team members to complete and return this assessment to our front counter and to continue self-monitoring at home before each practice.

As Covid Hotspots pop up around the country, we need our gymnastics family to do their part in limiting the spread of disease to keep our area safer.  This means you need to be vigilant when out in public maintaining physical distancing and wearing a mask when closer than 6 feet.

After we get this health assessment from all team members, we will be expecting parents to do their child's health screening questionnaire prior to coming.  We will continue to do the temperature checks.

Have you traveled outside the United States with in the last 14 days?
Have you traveled outside of Maryland to any known hotspots or to a State that is having a spike in Covid-19 cases within the last 14 days? IF so, you will need to self quarantine for 14 days or provide a negative Covid-19 test result prior to returning. We aim to keep our environment as germ free as possible.
Have you emailed in all summer travel plans, including weekend trips or day trips even if they don't require your gymnast to miss practice? Email should include: Date, location, will you be going out to eat, how will you social distance, will you be traveling in a large group, what measures will you be taking to prevent the spread of Covid-19.
Have you been in contact with anyone who has tested positive for Covid-19 without wearing PPE?
Has anyone in the household tested positive for Covid-19 or have any symptoms of Covid-19 in the past 48 hours including: cough, fever, chills, shortness of breath, body aches?
Has your child vomited in the past 48 hours?
As we begin working on some skills, it is sometimes necessary to spot our athletes either for safety reasons (such as the first back handspring, flyaways, or back tucks are done) or to help understand timing (such as front hip circle or kips). In these instances BOTH coaches and GYMNASTS will be required to wear their masks. Do you give permisson for coaches to spot your athlete as needed with these protocols?
Does your child have a well fitted mask that will not fall off during entry/exit of the gym and to wear during spotting for safety?
